Our work

Programs that meet people where they already are

Every program is free, led by trained neighbors, and designed to end in an action someone can take before the week is out. Bring one to your block, or join the next one.

Screening & awareness

Know Your Numbers

Free blood pressure, blood sugar and BMI checks hosted where people already gather.

  • Pop-up screening tables at barbershops, salons, churches, laundromats and neighborhood festivals.
  • Every reading written down and explained on the spot — no jargon, no waiting room.
  • A warm hand-off to a partner clinic when a reading needs follow-up, including help with cost questions.
Habits & longevity

The Everyday Five

Five simple daily actions that protect the heart, the brain and the years ahead.

  • Twelve-week neighborhood cohorts that meet once a week for twenty minutes — in person or on a group call.
  • Each week adds one action and keeps the last one, so habits stack instead of overwhelm.
  • Text check-ins and a paper tracker for members who would rather not use a phone app.
Education circles

Kitchen Table Talks

Small circles in living rooms and fellowship halls about the health we inherit.

  • Trained community hosts guide honest, judgment-free conversation about family health history.
  • Plain-language worksheets map heart disease, diabetes and cancer across generations.
  • Every circle ends with one thing to do and one person to tell.
Movement

Walk With Me

Neighborhood walking groups and a six-week step challenge with real people, not an app.

  • Volunteer-led walks at a set time each week — strollers, canes and grandchildren welcome.
  • Routes mapped to well-lit streets, parks and community gardens.
  • A six-week challenge that closes with a celebration hosted by a partner group.
